I figured out anyway, I was using a resolution of 1440 x 900, and had to lower to 1024x768...
Does anyone know the following: the patches MUST be installed one after another (Eg: 1.0.1.0 and 1.0.3.0) Or I just can install the last one 1.0.3.0, and if the crack works with this patch.

I forgot... I'm using Win XP with SP3, so everything should be fine, But still getting that fatal Error gta IV DD3D30 it's like the error that "Zith89" had, but with another code...
I get a Critical Error of GTA IV: DD3D30.
this happens when i try to launch the game. I followed all the steps in the correct order (Already pasted the crack, 1911.dll and the launcher), without the update 1.0.3.0, with this one installed gives another critical error of gta IV...
Anyone with the same problem around here??

Burn or mount
Close autorun, run install.exe from the ViTALiTY folder
Select a directory to extract the original installer
Run dxsetup & gfwlivesetupe in gamedirInstall
Copy the crack from the ViTALiTY folder
Run xlivestart to start the game
Create an xLive offline account if asked

you must install Alcohol 120% (found on the net), and it will read mdf n' mds files, you can "mount" them in a virtual drive created by the same software (Alcohol), or burn it, to DVD
